Application for Official Transcript

 

 

Students who wish to apply for official transcript should complete the attached form *Application for Transcript (HP42-TRN) and submit their application together with an application fee to the College Office.

 

* It is a fillable PDF form on which you can type information. You can also download this form in Learner Portal (under “Student Services Forms” of “My Resources”).

 

Application can be made by one of the following methods:

 

Application in person

Please print out the completed application form (Application for Transcript (HP42-TRN)) and submit the form to the College Office, 18/F in person during the #office hours. You should pay the application fee in cash.

 

# Office hours of the College Office:

Mondays to Fridays: 9:00 am – 5:30 pm

Saturdays: 9:00 am – 12:00 nn

Closed on Sundays and Public Holidays

 

Application through college email

Other than submitting your application to the College Office in person, you can apply for official transcript through college email following the below procedures:

 

Step 1: Complete the form Application for Transcript (HP42-TRN)

 

Step 2: Pay the application fee by ATM transfer. Collect the ATM Customer Advice from ATM

Please pay the exact amount of application fee by ATM transfer to the following account and collect the ATM Customer Advice from ATM when the transaction is completed:

The application fee is calculated according to the no. of copies applied for. Excess payment will not be refunded.

 

You are reminded that interbank transfer via online banking is not accepted.

 

Step 3: Email the application form and the scanned / photo copy of the ATM Customer Advice (with Student ID and name written on the Advice) using College email account

You need to use your College email account to send your application form and the scanned / photo copy of the ATM Customer Advice to exam.records@hkuspace-plk.hku.hk. Please write down your Student ID and name on the ATM Customer Advice before making the scanned / photo copy of it. You should keep the original copy of the Advice at least 3 months from the date of application as we may request you to submit it to us when necessary.

 

An acknowledgement will be sent to you by email upon receipt of your application.

 

Collection of transcript

The normal processing time for a transcript application is at least two weeks from the date of application. Due to the large number of applications received, the processing time will be longer if your application is submitted in the first two weeks after the release of the assessment results. Please check the notice posted on the Learner Portal nearer the time for the availability of transcripts and collection arrangement.

 

If you have chosen to receive the transcript by post, the transcript will be mailed to you according to the mailing address provided on the application form.
