Application for Official Transcript

(for the Assessment Results of Semester 2, 2023-24)

The assessment results of Semester 2, 2023-24 will be released on #26 June 2024 (Wed) (the date is tentative, please check the notice to be posted on the Learner Portal nearer the time for the confirmed release date) via Student and Course Record System (SCORE). Students can view their assessment results by generating an *unofficial transcript in SCORE starting from the release date.

# Except the assessment results of “Placement” (HPTH4017) whose release date will be on 22 July 2024 (Mon).

* Both official transcript and unofficial transcript list all the courses and grades a student has attained. Official transcript is a formal certificate of assessment results whereas unoffiical transcript is not. Please apply for an official transcript from the College Office if you need a formal one.

If you would like to apply for admission to the universities, you can submit the application forms and other required documents first before the deadlines and send the assessment results to the universities later once the results are available.

If you wish to receive the official transcript with the assessment results of Semester 2, 2023-24 shown on the transcript around early July 2024, please complete the attached form ^Application for Transcript (HP42-TRN) and submit your application together with an application fee to the College Office on or before 14 June 2024 (Fri). Students will receive the official transcripts in late July if they submit the applications after 14 June 2024.

Application in person

Please print out the completed application form (Application for Transcript (HP42-TRN)) and submit the form to the College Office, 18/F in person during the #office hours. You should pay the application fee in cash.

Mondays to Fridays: 9:00 am – 5:30 pm

Saturdays: 9:00 am – 12:00 nn

Closed on Sundays and Public Holidays

Collection of transcript

Students who submit their applications on or before 14 June 2024 will receive the official transcripts around early July. Please check the notice posted on the Learner Portal nearer the time for the release date of transcript and collection arrangement.

If you choose to receive the transcript by post, the transcript will be mailed to you according to the mailing address provided on the application form. Please note that in addition to the College Office’s processing time, extra 1-2 working days will be needed for the Post Office to deliver the mail, and the College Office accepts no responsibility for any loss or damage of transcripts during postal delivery.
