Application for Official Transcript

 

 

Students who wish to apply for *official transcript should complete the attached form ^Application for Transcript (HP42-TRN) and submit your application together with an application fee to the College Office.

 

* Both official transcript and unofficial transcript list all the courses and grades a student has attained. Official transcript is a formal certificate of assessment results whereas unofficial transcript is not. Please apply for an official transcript from the College Office if you need a formal one.

 

^ It is a fillable PDF form on which you can type information. You can also download this form in Learner Portal (under “Student Services Forms” of “My Resources”).

 

Application can be made by the following method:

 

Application in person

 

Please print out the completed application form (Application for Transcript (HP42-TRN)) and submit the form to the College Office, 18/F in person during the #office hours. You should pay the application fee in cash. (The application fee is calculated according to the no. of copies applied for.)

 

# Office hours:

Mondays to Fridays: 9:00 am – 5:30 pm

Saturdays: 9:00 am – 12:00 nn

Closed on Sundays and Public Holidays

 

Application through college email

 

Other than submitting your application to the College Office in person, you can apply for official transcript through college email following the below procedures:

 

Step 1: Complete the form Application for Transcript (HP42-TRN)

 

Step 2: Pay the application fee by “Bill Payment”.

Please pay the exact amount of application fee by “Bill Payment” through one of the following methods:

A set of Payment Guidelines which provides step-by-step procedures of using the above payment methods is attached for you to follow.

 

You are reminded that other payment methods sush as online banking transfer, FPS, credit card are not accepted.

 

The application fee is calculated according to the no. of copies applied for. Excess payment will not be refunded.

 

Step 3: Email the application form and your transaction record using your College email account

Use your College email account (your student number@learner.hkuspace.hku.hk) to send email to studentrecords@hkuspace-plk.hku.hk for making application for official transcript. In the email, you should provide the following information:

 

Email Title:

 

Application for Offiical Transcript – XXX (state your full name and SID)

 

Email Items:

 

(1)  Completed application form; and

(2)  Your transaction record (screen capture of your transaction record / scanned or photo copy of receipt from ATM / reference number of your transaction record).

 

An acknowledgement will be sent to you by email upon receipt of your application.

 

Collection of transcript

 

The normal processing time for a transcript application is at least two weeks from the date of application. Due to the large number of applications received, the processing time will be longer if your application is submitted in the first two weeks after the release of the assessment results. Please check the notice posted on the Learner Portal nearer the time for the release date of transcript and collection arrangement.

 

If you choose to receive the transcript by post, the transcript will be mailed to you according to the mailing address provided on the application form. Please note that in addition to the College Office’s processing time, extra 1-2 working days will be needed for the Post Office to deliver the mail, and the College Office accepts no responsibility for any loss or damage of transcripts during postal delivery.
